Each year the Melissa Cook Memorial Foundation awards several scholarships to deserving individuals to further their learning, with preference given to students from Lake County, IN. The number and size of the grants will be determined by the Foundation’s Scholarship Committee, whose decision will be final. The award may only be used to help defray tuition and room and board costs at the school of the Applicant’s choice, and will be sent directly to the college. Continuing grants will be available, contingent upon the Applicant remaining in Good Standing.
To apply for a scholarship, the following steps must be taken:

Complete the attached application. Fill in all information
applicable to your situation. If you have a question, provide
more information rather than too little. If necessary,
additional pages may be attached. Be sure to attach all
supporting documentation requested.

Submit the following: official copies of all transcripts of the
entire high school enrollment; two letters of reference as
stated in the application; copy of your family’s 1040 with all relevant schedules and
W-2’s for the most recent tax year. Please note: The
application and supporting documentation must be mailed as
a complete package. |

Please type or neatly print all application information.
Illegible submissions will not be considered.
