Sponsor The Melissa Cook Memorial Foundation Fundraiser (“Fundraiser”) is sponsored by the Melissa Cook Memorial Foundation (“Sponsor” or "MCMF"), a tax-exempt non-profit, which the IRS permits to operate as a public charity. Contact information for MCMF can be found on the Contact Us page on http://www.melissacook.org. Eligibility The Fundraiser is offered to all natural persons who are 18 years old or older. Employees, officers, and directors of Sponsor; affiliates; advertising, promotion, and fulfillment agencies; and legal advisors, and their spouse/children and persons living in the same household, are not eligible to participate in the Fundraiser. Void where prohibited. All federal, state, and local laws and regulations apply. How To Enter When You Donate Suggested donation is $25.00. Each $25.00 thereafter will provide an additional entry. For example, if you donate $100.00, you will be entered 4 times. You can enter as many times as you wish during the applicable dates. Enter online at http://www.melissacook.org or send an entry using a business size envelope and include a piece of paper, legibly bearing your name, date of birth (you must be 18 years or older) address, e-mail address, phone number, and signature to P.O. Box 426 Arlington Heights, IL 60006. Prize Two (2) tickets/press box passes to the Notre Dame vs. Pittsburgh football game ("Game") on October 09, 2010; two (2) roundtrip passes that are good for travel between any two cities within the Southwest Airlines system; a VIP Tour of Notre Dame Stadium; one (1) parking pass to the Game; one (1) two night stay at a local hotel for Friday October 08 and Saturday October 09; one (1) gift certificate to dinner. Selection of Winner Winner will be determined by a random drawing from each eligible entry on September 1, 2010. All decisions are final and binding. The winner will be notified by fax, e-mail, telephone, or regular mail. Sponsor is not responsible for and shall not be liable for late, lost, misdirected, or unsuccessful efforts to notify winner. Winner Winner may be required to sign and return a release of liability, declaration of eligibility, and, where lawful, a publicity agreement, in connection with accepting prize, all within 14 days of notification. By accepting and/or using prize, winner agrees to the use of his or her name, voice, or likeness for the purpose of advertising or promotion without further compensation, unless prohibited by law. Winner cannot resell any portion of the prize package. If a selected winner cannot be contacted, is ineligible, fails to claim a prize, or fails to timely return the completed and executed affidavit and releases as required, prize will be forfeited and an alternate winner will be selected. Conditions By entering, participants agree to be bound by these Rules & Regulations and the decisions of Sponsor, which shall be final. Sponsor, agents, and representatives of Sponsor; affiliates; advertising, promotion, and fulfillment agencies; and legal advisors, are not responsible for and shall not be liable for (i) late, lost, delayed, damaged, misdirected, incomplete, illegible, unintelligible, or postage-due entries; (ii) telephone, electronic, hardware or software program, network, Internet, or computer malfunctions, failures, or difficulties of any kind; (iii) failed, incomplete, garbled, or delayed computer transmissions, (iv) any condition caused by events beyond the control of the Sponsor that may cause the Raffle to be disrupted or corrupted; (v) any injuries, losses, or damages of any kind arising in connection with or as a result of the prize, or acceptance or possession, or use of the prize, or from participation in the Fundraiser, or (vi) any printing or typographical errors in any materials associated with the Fundraiser. Sponsor reserves the right, in its sole discretion, to suspend or cancel this Fundraiser at any time if a computer virus, bug, cyber-vandal, or other technical problem corrupts the administration, security, or proper play of the Fundraiser. By Participating In The Fundraiser Participants agree to release and hold Sponsor and the employees, officers, directors, agents, and representatives of Sponsor; affiliates; advertising, promotion, and fulfillment agencies; and legal advisors, harmless from any kind of losses, damages, rights, claims, and actions of any kind in connection with the Fundraiser or resulting from acceptance, possession, or use of any prize, including without limitation, personal injury, death, and property damage, and claims based on publicity rights, defamation, or invasion of privacy. Winners List and Rules & Regulations To obtain the name of the Fundraiser winner send a request along with a self-addressed, stamped envelope P.O. Box 426 Arlington Heights, IL 60006. Requests must be received by September 31, 2010. To obtain a copy of these Rules & Regulations send a self-addressed, stamped envelope to the address mentioned above. No Purchase Necessary You Need Not Be Present To Win Nor Donate To Enter Or Win You need not be present to win. In lieu of an Entry Form containing a donation, you may obtain a Decline to Make Donation Fundraiser Entry Form by writing to: Decline to Make Donation Fundraiser Entry Form, P.O. Box 426 Arlington Heights, IL 60006. Use a business size envelope and include a piece of paper, legibly bearing your name, date of birth (you must be 18 years or older) address, e-mail address, phone number, and signature. State on such paper that you decline to make a donation to MCMF. Include a self-addressed, stamped, business size envelope. Each request must be mailed separately. Limit one request per envelope. While the supply lasts, MCMF will mail to you your free Decline to Make Donation Fundraiser Entry Form. Free tickets defeat the charitable fund raising purpose of the fundraiser and are not encouraged by several States. MCMF is not responsible for late, lost, misdirected, mutilated, or postage due entries, or request for entry forms. Purchases of raffle tickets are considered purchases of a chance to win a prize; therefore, the amount paid to purchase raffle tickets is not deductible in whole or in part as a charitable contribution. |
